The first two are the ones to act on. Landing next to someone repeatedly means you are being shown to the same person at the same moment, which is the only definition of a competitor that changes what you write in your metadata.
A "Similar app" badge appears when the store itself lists the app as similar to yours. It is a corroborating signal, not the reason a suggestion ranks highly.
Suggestions never include your own apps, and an app disappears from the list the moment you track it. Each card links out to the store listing so you can look before you commit a slot.
Adding one yourself
You can always pick the competitor. Choose which of your apps it belongs to, then either search the store by name or paste the listing URL directly. The country and language default to the market of the app you selected, which matters: a competitor is tracked per market, exactly like an app across markets.
Pasting a URL is the reliable route when the name is generic or the app is hard to find by search. Both App Store and Google Play links work. A search that comes back empty is nearly always the market rather than the spelling, which I cannot find my app when adding it to Applyra goes through.
